The appropriate treatment plans depend on the precise interpretation of the diagnostic images and the examinations obtained with the latest equipment in ocular diagnosis. Multimodal diagnostic imaging studies, such as ultrasound, fluorescein angiography and optical coherence tomography (OCT), corneal topography, pentacam, and biometrics, are essential tools for Dr. Gutierrez to determine the cause of the problem and plan the correct procedure for Visual rehabilitation of the patient and long-term eye health. It uses highly technical instruments to diagnose extremely delicate tissues in the incredibly small space of the eye cavity.

Advanced cataract surgery, Dr. Gutierrez removes opaque (crystalline) natural lens (cataract) through a 2 mm microincision and replaces it with high-tech intraocular lenses (IOL) with world-renowned products such as ALCON; in this way, those patients who suffer from farsightedness, astigmatism, myopia and/or presbyopia can reduce and/or eliminate the dependence on the use of glasses for daily activities of near and intermediate vision such as the use of cell phones, computers, tablets, paint, among others; and far vision activities such as driving, playing golf, movies, sports, etc.

In minimally invasive retinal surgery, the retina is a layer of cells in the back of the eye that detects light. The information obtained through the light stimulus is transformed into impulses that are sent to the brain for interpretation through the optic nerve, which allows us to see. The macula is in the center of the retina and helps to see things in detail and definition. When the macula is affected by a disease, injury, or age, central blurred vision, a spot in the center of vision, or image distortion (metamorphopsia) may occur. Metamorphopsia will make patients see linear objects, such as lines in a grid, look curved or rounded, this symptom is usually perceived by the patient during near vision activities such as reading, where they will perceive distortion of the letters or They should skip during reading.

Macular hole, epiretinal membrane, Dr. Gutierrez is an expert surgeon in macular surgery, using advanced techniques such as internal limiting membrane flap for the treatment of large macular holes. Macula surgery, surgery is performed through minimally invasive vitrectomy (25 Gauge or 27 Gauge caliber) for early rehabilitation and post-operative comfort; It is an emergency surgery since the anatomical and functional success will be related to the diagnostic vision, macular hole size, and evolution time (time of onset of symptoms).

Age-related macular degeneration (AMD), is a degenerative eye disease that affects people over 60 years of age, causing visual disturbances in central vision. Smoking, old age, family history of macular degeneration are considered the main risk factors.

There are two types of AMD:

  • Dry or atrophic age-related macular degeneration (AMD): Affects about 80% of patients diagnosed with AMD. The deposits that accumulate in the area atrophy the macula and cause the patient to gradually lose vision in the central area of his visual field.
  •  Wet or exudative age-associated macular degeneration (AMD): It is characterized by the growth of new abnormal vessels (choroidal neovascularization) and is very fragile under the macula. These immature vessels leak blood and fluid, causing the patient to report a sudden or short-term visual loss in their central field of vision. Timely treatment and intravitreal injections with anti-angiogenic drugs are required to reduce sequelae and preserve long-term vision.

Retinal detachment is a separation of the retina by sub-retinal fluid. The detachment of the rhegmatogenous retina is the most frequent and occurs associated with a tear or rupture of the retina, which allows the liquefied (degenerated) vitreous to pass through the sub-retinal space. A detached retina affects nerve cells, which can be permanently damaged, and must be repaired quickly to prevent vision loss. The main risk factors are myopia, eye trauma, and posterior vitreous detachment. The vitreous is suspended in the back of the eye between the retina and the lens; The vitreous is transparent gelatin composed of 99% water, collagen, hyaluronic acid, glucose, sodium, potassium, and proteins. It is important to see a retina specialist right away to rule out a retinal detachment.

  •  Flying flies appear for the first time or a new one appears (myodesopsia).
  • Suddenly flashes or flashes of light are seen in the absence of light stimulation (photopsia).
  • Loss or reduction of the peripheral visual field is referred to as the presence of a shadow in the mink field that progresses to the center of vision.
  • When a recent or old eye trauma has occurred.
  • When you have myopia because retinal holes and tears are much more frequent in patients with myopia.
  • History of retinal detachment, since there is a 4-fold greater risk of developing retinal detachment in the contralateral eye (healthy eye).

Diabetic retinopathy is a complication of diabetes that affects the eyes by damaging the blood vessels of the retina in the back of the eye. If you have diabetes, you may not notice any change in your vision at the beginning. But over time, diabetic retinopathy can cause an irreversible loss of vision. The condition can develop in anyone who has type 1 or type 2 diabetes. The progression of diabetic retinopathy will be related to the control of blood sugar, high blood pressure, and triglycerides.

Refractive surgery, is the set of surgical techniques that aims to eliminate or minimize refractive defects such as myopia, hyperopia, and astigmatism. It can be applied through current laser techniques (laser refractive surgery) with Femtosecond (Femto-LASIK or iLASIK) used in the OCULAR SURGERY CENTER, this technique, replaced LASIK, which is performed by means of a knife with an instrument of microkeratome. Public service and caring for the less fortunate is a way of life in the Latin American culture.


Dr. Mario Gutierrez is currently the Head of the Retinal Surgery Department and post-graduate studies program at the Pan-American Institute Against Blindness – (IPC). In this capacity at the foundation, Dr. Gutierrez develops academic curricula and oversees clinical research. The IPC is a non-profit organization that provides specialized ophthalmology care and blindness prevention for people with limited resources. They are headquartered in Guatemala City and have regional clinics in rural communities of Sacatepéquez, Escuintla and Huehuetenango to provide low-cost vision care to local residents.
